📋 Description

• Per Diem/PRN Medical Social Worker in Eastern Jacksonville/ Beaches area, FL
• Visits on a per visit basis in home health settings
• 5-10 visits weekly; home health experience not required but preferred

🎯 Requirements

• Must be licensed by the FL Board of Social Work Examiners at LMSW or LISW level
• Master of Social Work with at least 2 years of community-based experience
• CSWE accreditation demonstrated by transcript or degree
• Home care experience a plus
• Strong reading, writing, and English communication skills
• Ability to work independently and manage time
